What was the first aortic arch repair in Cyprus with Nexus Tre?

First endovascular repair in Cyprus of an aortic arch aneurysm, following chronic type A dissection, with the Nexus Tre endograft. The procedure was performed in the Hybrid operating room of the Aretaieio Hospital in Nicosia.

Nexus is a branched endograft designed specifically for the aortic arch, the most demanding segment of the aorta.

In the aortic dissection the middle layer separates from the outer layer and the aorta acquires two lumens. The aneurysmal degeneration that follows often affects the aortic arch.

What is different about the Nexus Tre?

The Nexus Tre is a version of the Nexus system with three branches, designed for anatomies where flow has to be preserved in more vessels of the arch. The choice between the versions is made on the basis of each patient's anatomy, as shown on CT angiography.

Why does a first application in a country matter?

The availability of a technique in a country depends on whether there is a surgeon trained in it and a centre with the necessary infrastructure. A first application means in practice that patients in that country gain access to a treatment that until then required travelling abroad.

How is it linked to aortic dissection?

Aneurysmal degeneration after a chronic type A dissection is among the most common indications for surgery on the aortic arch. How is the method chosen?

The decision is not standardised. It depends on the anatomy of the aneurysm, the age, the general condition and the history of previous operations. It requires a vascular surgeon with experience in both techniques and a centre with the appropriate infrastructure, so that the treatment is adapted to the patient rather than the patient to the available technique.
